Release of Liability and Hold Harmless

Acknowledgement:

I acknowledge and understand that reasonable care will be taken by the supervising personnel to insure the safety of my child. It is absolutely essential, however, that you as parent or legal guardian, give written permission for your child to participate in these activities. Therefore, please complete the remainder of this form and sign below.

Release and Waiver:

In consideration of my child participating in Junior Lifeguards, I hereby release World Academy of Safety and Health (WASH) LLC, World Academy of Safety and Health (WASH) International LLC, Lifeguarding Classes LLC, World Academy of Safety and Health Educational Services LLC and their agents, heirs, employees and principals, volunteers, and all other associates of and from any and all liability, claims, demands, actions, and causes of actions whatsoever, arising out of or related to any loss, damage, or injury that may be sustained by my child or children.
